Montgomery-Åsberg Depression Rating Scale

A 10-item clinician-administered scale measuring severity of depressive symptoms. More sensitive to treatment-related change than the HAM-D and less weighted toward somatic symptoms.

Patient Health Questionnaire-9

A 9-item self-report measure of depression severity keyed to the DSM-5 criteria for major depressive disorder. Widely used for primary-care and specialty screening, treatment response tracking, and remission monitoring.

Generalized Anxiety Disorder 7-item scale

A 7-item self-report measure for generalized anxiety disorder, with strong performance as a screener for panic disorder, social anxiety disorder, and PTSD as well. Sensitive to change with treatment.

Columbia Suicide Severity Rating Scale — Screener

A 6-item screener for suicidal ideation and behavior. The overall severity is driven by the *most severe* item endorsed, not a sum; a positive on items 4, 5, or 6 warrants immediate safety triage.

Young Mania Rating Scale

11-item clinician-rated scale for severity of manic symptoms. Items 1–4, 7, 10–11 are scored 0–4; items 5, 6, 8, 9 are double-weighted 0–8. Total 0–60.

Clinical Institute Withdrawal Assessment for Alcohol, revised

A 10-item clinician-administered scale quantifying severity of alcohol withdrawal. Drives symptom-triggered benzodiazepine protocols and helps decide level of care.

Clinical Opiate Withdrawal Scale

An 11-item clinician-administered scale quantifying severity of opioid withdrawal. Commonly used to time the first dose of buprenorphine during induction.

Abnormal Involuntary Movement Scale

A clinician-administered scale for screening and tracking tardive dyskinesia. The movement-severity subset (items 1–7) is scored here; a composite sum and any single-area score of moderate or greater should prompt further evaluation.
